I was scared to DEATH of E.T. He creeped me out like nothing else.
"Animal Antics", because the Simitar Logo and red "clock" font at the beginning were scary enough, and then the Spider that appeared around the beginning of the actual content was enough to have me run off crying and hide in a closet until that particular segment was over...
I used to turn down the volume and go get a snack until the FHE logo was over on some holiday tapes. On the same ones I used to try and stop it before the Rankin Bass logo came on. Also, the white screen of death is the main reason why I never fall asleep watching a video anymore, even if it's a DVD.

Tums commercials used to scare me "TUM TA TUM TUM TUMMMMM" and the old old commercial for the "viper" car alarm or whatever.
For some reason the Sledgehammer theme used to scare me as well. Unsolved Mysteries and Are You Afraid Of The Dark theme songs both creeped me out.
