For me it was the face melt from Indiana Jones and the Raiders of the Lost Ark (1981).

Cohaagen dying at the end of Total Recall. I'm not sure it was really scary, but it was definitively a bit creepy or shocking to me. I do remember one thing that truly scared me from this movie as a kid, the scene where Quaid is using a woman's mask and it starts to malfunction and then starts quivering and stuff. The expression on that woman's face scared me I don't know why.

I don't know if this counts or not but that scene from the movie The Dark Crystal where the little podling gets strapped into the chair and is forced to gaze into the beam of red light and his living essence is drained from him. That scene scared me so bad that I didn't even finish the movie after that I was so scared. Especially when they talk about doing it to the gelflings to make them young again. I was afraid that Kyra and Jen were going to suffer the same fate.

I don't know if I would consider it scaring me, but it was somewhat disturbing...but when I was around 11, I was watching this one episode of Johnny Sokko and his Flying Robot, called "The Terrifying Space Mummy", which at the beginning shows the "space mummy" poisoning the city water supply, then immediately showing a young boy and then several kids drinking a glass of water from their kitchen faucets and dropping dead, with their faces rapidly distorting and turning mummified.

I remember watching it on TV once and I was so unnerved.

Judge Doom's death from Who Framed Rodger Rabbit 1988
It was pretty damn morbid for something out of a PG-13 Movie.

Both Dennis Nedry's and Robert Muldoon's deaths in Jurassic Park messed with me. I was never really attached to the characters, but their deaths seemed so brutal in that movie and they way they were filmed made them worse. You never saw anything but a glimpse of the attack and then there was screaming.

When the Joker kills that one gangster with his electric joy buzzer in the first Batman, roasting his flesh off. It took me years before I could watch that scene again.

Speaking of Batman death scenes, the scene where young Jack Napier killed young Bruce Wayne's parents kind of corrupted my mind.

The beginning of Friday the 13th. It was a long time before I saw a Friday the 13th movie again.

I'll agree with The Raiders face melting scene. I saw that in the theater and had to close my eyes.

I also remember that scene from The Children (1980) where a woman is burned/shriveled to death in the cemetery when a possessed child touches her. I saw that on TV in the mid-80s and it disturbed me.

Bridge to Terabithia - Leslie Burke had hit her head on some rock, bringing Jess Aarons' new friend to her death because Leslie drowned in the water.

I always thought that was silly, what with the prop skeleton.

Now, Dr. Jason Woodrue killing Pamela Isley in Batman and Robin? I'm ashamed to admit it, but that actually kinda scared me as a kid.
Yeah, it's Yuuki. Got a problem with that?

Lena's death in the Super Mario Bros. movie where she's fried and fossilized by the meteor's energy leaving nothing but her skeleton. And then the jaw suddenly drops open.

Artax's death wasn't the scariest, but it was sad and has stayed with me my whole life. I feel like that movie and death scene in particular scarred a whole generation.

Jango Fett being decapitated in Attack of the Clones... funny thing is, I had no problem with it the first time I saw the movie, but when I discovered Swayzak from Toonami (who had a New Zealander accent like Jango), I suddenly became obsessed with Jango and couldn't STAND to see him get killed. Still kinda puts a tear in my eye to this day.

There were 2 death scenes that were scary.

Mr. Flanders, a character from The Simpsons, saw Mrs. Flanders falling off the bleachers to her death.
Ash Ketchum and Pikachu were completely clueless when they were ghosts after a serious collapse had wiped them out during a episode of Pokemon (the animated series).
