Mortal Kombat 5 ( I believe) where they are falling down a hole, counting down to 10. If you didn't make it he/she would scream and hit spikes, oozing blood on the bottom.

    Music was creepy.

For some reason I always found the Maximum Carnage game over screen to be somewhat unnerving.

Bart's Nightmare (Genesis) - Bart wakes up and he sees his grade. What's worse is an F and the family getting rude remarks to Bart as his paper and the player's final score are posted on the refrigerator!

I was never scared of them, but I never liked getting them (who did?)
The one from DKC3 was a little weird, though, but not very scary

    I've got one for you!

In the Spectrum/Commodore 64/Amstrad game Grange Hill (based on a British school-based TV show of the same name), there is a drug dealer somewhere in the game (relating to the anti-drug aspects from the show). You're supposed to refuse the drug dealer's offer of drugs, but if you accept his offer of drugs, the game ends, leaving you with this message:

There is an empty look in his eyes as he snatches the money from your hand. His face is pale and drawn, his body thin and unfed. He steals to keep his habit, and makes addicts of children. He is dead, and soon, you will be too.

Sonic CD's American Version was pretty scary.
The Japanese/European soundtrack was more "Awww...." while the American just scares me.

How come no one mentioned Banjo-Kazooie's game over screen? With Tooty becoming that ugly green mutant monster? That scared the hell out of me!

Haha you're totally right, that was pretty bizarre. I seem to remember being scared of the Friday The 13th game over screen. Nothing like reading a sentence that says you and your friends are dead, especially when you're still alive to read it.

I also hate continue screens. It's like I lost alright, don't make me do it all over again!!

Rolling Thunder had an odd game over screen. It was similar to Gannon laughing, but it was a wierd alien looking dude.
