The thing is, SMB2 was NOT from Super Mario Bros. all the elements for the game were taken from Doki Doki Panic, because NoA believed that the real SMB2 was too hard for Canadians & Americans...so we ended up getting a bastardized game with Super Mario characters tacked onto it. ...yes and no. Doki Doki Panic really began as a rejected Mario prototype. Stars, coins, POW blocks, some sound effects, and warping between levels all originated from Super Mario and were part of the project from the very beginning. The goal was to experiment with making the action and movement mostly vertical instead of horizontal. To do something different for the sequel. Ultimately, they decided it wasn't fun enough and put it aside.
This also explains why Zelda 2 was so different than Zelda 1. Nintendo likes to start with a style or control system, and then decide what kind of atmosphere or characters to put with it.
Later, a tech expo called Dream Factory asked them to make a game that they could use for promotion and gave them some Fuji TV characters. Nintendo based it on that prototype, added a lot of horizontal action, and tacked on the Fuji characters that were requested. The game was surprisingly popular, leading to the decision to release it in the west, but nobody would know those characters in this market. The obvious choice was to ditch them and return Mario to the starring role.
The little prototype that could was also influenced by some things that they wanted to do with Mario, but couldn't at the time because of technical limitations. Just as Miyamoto wanted a Yoshi-type in the original SMB, he wanted simultaneous multiplayer in the sequel. He wanted two players to be able to pick up and throw each other. (Finally able to do this in New SMB Wii.) That didn't work out, but they kept the pick up and throw mechanic for enemies and vegetables.
