I've had had 3 systems die, but 2 of them I already replaced ages ago. My columbia home entertainment system (ie, a rebranded coleco atari 2600), my old front loading NES, and my game gear.
I got an atari 2600 from a friend of a friend about a dozen years ago, and I got a top loading NES just as my front loader was on it's last legs. The game gear is the only system which i don't currently have a functional version of. Then again, I have an emulator & roms, so it's all good.

I never really had any of my consoles died on me,I just abanddon them. (sigh) I have no idea where my Atari 2600,Intellivision and Colecovision ended up. My Mom gave those away when I was in college. I gave away my SNES and the power supply of my Genesis burned up and I can't remember what happened to it. However,I have stored in my basement an old TGF-16 I bought off eBay,my old Saturn,my original PSX (which I had repaired like 5 times),ye olde N64,my 9/9/99 DC, my second PS2 (I dropkicked the first one},the cute Gamecube and last -but hardly least- the almighty original Xbox (complete with Dukes).

My Sega Genesis (listed below as Genesis #1) which I'd grew up with with had broken down. The one I got a few years later (Genesis #2) was not broken, but stolen - system, games, controllers, etc. - by one of my family's former landlords. Those brown-nosing landlords never gave it back to me, and I hope they get fired. However, I miss both of those two Genesis systems I've had.

                  Genesis #2 - 2003-2009

Man DS Lites are fragile. I have a SNES from 1991-1992 ish and it still works well now.
