The interrogation room in Resident Evil 2. Scared me senseless.

I think Doom 1 and 2 scared me more times than Doom 3, primarily because in Doom 1 and 2 I couldn't hear their footsteps and gauge where they are. That, and when they teleport in they're ready to kick my ass. The Demons and Revenants had some rather frightening sounds.

I used to feel bad about running over the guards with the tank in Goldeneye, mainly because of the screaming and squishing and how awful it must feel to be in their positions. Usually I'd drive slowly, either to blow them away with the cannon (like that's a better fate) or knock them down with the front of the tank.

The Battlefield 2 demo gave me quite a few sudden jerks. Being in the middle of an artillery strike is intense, as is getting killed by a nearby grenade. I remember playing as a Support character with my machinegun deployed over a hill. I was taking a few shots at an enemy when this bigass tank comes up in front of me. I get up very quickly and run off to the side of the tank, while yelling "Oh shit! Oh shit!" the entire time.

Natural Selection makes me paranoid about things hanging from ceilings and hiding in dark corners.
